In order for students to receive medications during school hours without parents/guardians being present, either in the health office or self-carry, we need to have the correct and current documentation on file.

Self-Carry only available for 6th-12th grade and only emergency asthma or allergy meds may be self-carried.
Update Health Records & Forms for 2024-25
For the 2024-25 school year, all necessary forms can be found on our School Nurse page.
Please make sure the “care plan” for the appropriate chronic medical condition is what you have your child’s medical provider fill out before school. This will ensure we have all the paperwork we need to be ready for the school year.
Medication authorizations and Emergency Health plans must be renewed every school year and signed by the parent/guardian and the child’s medical provider.
Please return completed forms as well as the prescribed medication in the original container at the start of school in August.
